Pursuant to Michigan Executive Order No. 2020-21, commonly known as Stay Home, Stay Safe, Saves Lives, my employer INSERT COMPANY NAME, has designated me to be part of the “minimum basic operations” team as provided for in Section 4 (b) of the Executive Order.  

This authorization covers the period from March 24 to April 13, 2020 and requires me to travel between my home and our business location at INSERT BUSINESS ADDRESS.
